Actually, he was only the fifth -- for some reason, I think Augustus is counted as the first (perhaps the first to officially adopt the title?)
On the other hand, Nero was the sixth (and last, I believe) Caesar; there was Julius, Augustus, Tiberias, Caligula, Claudius, and then Nero. I don't know if this is an allowed interpretation, but could the 666 be emphasising the number 6 through repetition? Then the number of his name being 6 has a very prosaic meaning -- the sixth Caesar.
